FRESHMEN
- Report Card (Form 138)
- Certificate of Good Moral Character
- Two pcs. 1x1 and 2x2 pictures w/ red back ground
TRANSFEREES
- Transcript of records or true copies of grades
- Honorable Dismissal
- Certificate of Good Moral Character
- Letter of Application of the Student
- Letter of Parental Consent
- Two pcs 1x1 and 2x2 pictures w/ red background
FOREIGN STUDENTS
- Photocopy of the Alien Certificate of Registration
- Previous Study Permit (If Transferee From Other Philippine Schools)
- Accomplished Personal History Statement
- Certificate of Bank Deposits
- Affidavit of Support
- Birth Certificate of Passport
- Transcript of Records
- High School Card
- Seven (7) Copies of 2x2 Pictures
- Two (2) Copies of 1x1 Pictures (Items 3-10 are needed for Student Visa Application)
Note: Continuing Foreign Students must meet the same requirements as of Filipino students in addition to the above-mentioned requirements.
PROCEDURES
- Apply for the entrance exam for freshmen and qualifying exam for transferees.
- Proceed to the College Dean’s Office for an interview.
- After the interview, pay the necessary fees at the Cashier.
- Present the proof of payment to the Dean of the College for the scheduling of the entrance examination/qualifying examination.
- After passing the entrance examination, proceed to the Registrar’s Office for the enlistment of the subjects to be taken during the semester.
- After paying the fees and enlistment of subjects, you will be officially enrolled.
- Get a copy of your registration form which shows all the subjects that you will need to take during the semester/trimester.
