· Certificate of Good Moral Character
· Accomplished Registration Form
· Pictures 4 copies colored 1x1
Transferees
· Transfer Credential (Honorable Dismissal)
· Certified True Copy of Transcript of Records or Certificate of Grades
· Passed the Entrance Exam and Interview
· Accomplished Registration Form
· Pictures 4 copies colored 1x1
PROCEDURE
1. The admission requirements will undergo evaluation and verification.
2. Scholarship examination will follow next (optional).
3. The applicant will undergo an interview to know which course or strand he/she will take and to discuss the tuition fees and other expenses.
4. Pay tuition fee.
5. Enrollment will then be formalized.
SENIOR HIGH SCHOOL PROGRAMS

· PSA Certified Birth Certificate
· Certificate of Good Moral Character
· Photocopy of Grade 10 Report Card with LRN / E-SID
· Pictures 4 copies colored 1x1
· Certificate of JHS completion
· ESC Certification Letter from JHS (for ESC grantees)
· QVR Certificate (for Private school JHS completers)
