INCOMING FRESHMEN
- Report Card (Form 138) or Permanent Record (Form 137)
- Photocopy of Birth Certificate
- Certificate of Good Moral Character
- 3 copies 1.5 x 1.5 ID pictures
- Entrance examinations conducted by the Guidance Office
RETURNING STUDENTS
- School Clearance
- Accounting Clearance
- Final Grades obtained during the last semester/term attended
TRANSFEREES
- Transfer Credentials
- True copy of grades earned from previous school
- Certificate of Good Moral Character
- Photocopy of Birth Certificate
- 3 copies 1.5 x 1.5 ID pictures
- Qualifying examination (for Nursing)
FOREIGN STUDENTS
New students (coming from their home countries):
- Student visa
- Electronic card (Bureau of Immigration)
- Original Transcript of Records (authenticated by the Philippine Consulate from their home country)
Transferees (from other schools in the Philippines):
- Student visa or visa extension
- Electronic card
- Transfer Credential / Honorable Dismissal
- Certified True Copy of Records
- No objection to transfer
- Entrance examination

Resident / Old Students WITHOUT Disciplinary Cases
- Report Card (Form 138)
- Report of Summer Grades for students with back subjects/deficiencies
- Clearance from the Accounting Office, Registrar and Principal for a student with accountabilities
Resident / Old Students WITH Disciplinary Cases
- Memorandum of Agreement (MOA)
- Report Card (Form 138)
- Report of Summer Grades for students with back subjects/deficiencies
- Personal appearance of both student and parent/guardian
- Clearance from the Accounting Office, Registrar and Principal for a student with accountabilities
Incoming Freshmen
- Report Card (Form 138)
- Certificate of Good Moral Character from the Principal or Guidance Counselor
- Personal Appearance of both student and parent/guardian for an interview with the Principal
Transferees
- Report Card (Form 138)
- Certificate of Good Moral Character from the Principal or Guidance Counselor

Foreign Transferee Students
- Student study permit and academic record duly evaluated by the Department of Education
