Enrollment Procedures for Regular College Applicants
 

Enrollment Procedure for College Applicants under the SOCIALIZED TUITION AND MISCELLANEOUS FEES PROGRAM:

1. Proceed to the MPO Office after receiving an SMS confirmation of being qualified to the program. Ensure that all requirements needed for enrollment were accomplished and/or submitted (e.g. Original Form 138, 1pc2x2 picture, Good Moral Certificate, Photocopy of NSO/PSA Birth Certificate, entrance examination) among other requirements of the STMF Program.
2. Secure all submitted document and forward it to the Admissions Office.
3. Proceed to the SAS Office for the Drug Testing Fee (P150.00)
4. Proceed to the Mobile Drug Testing (Mini Bus Near Cassiciacum Student Center)
5. Proceed to the Health Services Unit for Medical Check-up
6. Proceed to the Registrar's Office for Assessment.
7. Proceed to the Finance Office for initial payment.
8. Proceed to the Main Library, 2nd floor of the Main Building, for ID picture taking.
9. Proceed to the 2nd Floor, Café Monica, to purchase/order uniform, ID lanyard and ID Protector.

Requirements:
Freshmen

1. 1 pc 2x2 picture
2. Good moral certificate
3. Original grade 12 card
4. Photocopy of PSA birth certificate
5. Entrance examination (php. 300.00)
6. ALS/PEPT certificate (if applicable)

Transferees

1. 1 pc 2x2 picture
2. Good moral certificate
3. Honorable dismissal
4. Photocopy of psa birth certificate
5. Entrance examination (php. 300.00)

• ESC Certificate (If applicable - For Private Completers)
• Reservation Fee (P500.00 - For Private Completers)
