  • Official Transcript of Records (TOR)
  • Notarized Certificate of Sea Service (Company) signed by Master or C/E with ship’s stamp and the following information:
    • Vessel name and type
    • Gross tonnage
    • Ship’s port of registry
    • Propulsion power in kilowatt
    • IMO number
    • Length of service and the position/capacity held
  • Notarized MARINA Certificate of Sea Service
  • PCG (Philippine Coast Guard) - Master’s Declaration of Safe Departure (MDSD) 
    • Certified True Copy
    • For 12 months of seagoing service: Submit 12 MDSD, 1 per month on board.
    • For 36 months of seagoing service: Submit 36 MDSD, 1 per month on board.
  • Crew list from ship/ships served with Ship’s Stamp (For Domestic/inter-island vessels)
    • For 12 months of seagoing service: Submit 12 crew list,1 per month on board.
    • For 36 months of seagoing service: Submit 36 crew list,1 per month on board.
    • In IMO FAL Form no. 5 Format
    • Signed by Master, Signed by PCG(Phil. Coast Guard) officer, PCG stamped and dated.
  • Immigration stamp on Seaman’s Book/Philippine Passport, upon departure and arrival in the Philippines (For Overseas or Foreign going vessels)
  • Seafarers’ Identification and Record Book (SIRB) or Seaman’s Book (Entries from every vessel served.)
  • Notarized Company Certificate of at least performed 6 months Bridge or Engine Watchkeeping duties from ship served.
  • Ship’s Particulars from every ship served (Certified True Copy)
  • Training Certificate issued by Training Center and Certificate of Proficiency (COP) issued by Marina
    • Competence in Security Awareness (Certified True Copy)
  • Company Certificate of at least performed 6 months Bridge or Engine Watchkeeping duties from ship served
  • Training Record Book (TRB)
    • Registered at MIT/STO Office
    • All pages stamped (school/STO stamp)
    • Properly filled, signed and dated Project Work, Shipboard Activity Workbook, and Project Supplement
  • Daily Journal (for apprenticeship)
  • Passed the computerized examination conducted by OLT Maritime Training Review & Assessment Center
  • Passed the Oral Assessment conducted by MIT Maritime Assessors
  • 4 pcs. 2x2 ID picture
    • Proper haircut
    • White uniform with shoulder board w/o stripe
    • Full name with middle initial

Misamis Institute of Technology (MIT), Inc. is a private school in Ozamiz City known for its maritime programs.
