- Form 137 (High School Report Card)
- Photocopy of Birth Certificate
- Certificate of Good Moral Character
Procedures
- Secure an application form from the Administration Office at the ground floor, PRIFI Building. The applicant will take the entrance examination on a preferred date.
- Applicants aspiring for scholarship must secure the appropriate form. An average grade of 83% is required.
- After having passed the entrance examination, the aspirant must submit the requirements
