Requirements

Have obtained an average of at least 2.0 or its equivalent with no failing grade in any subject
Accordingly have their academic grades evaluated by the Dean or Admission Services
Submit the following original and photocopied documents to the Admission Services
Letter of application
Medical clearance from the University Clinic, and
Transcript of Records with remarks as Graduated
Pay the Admission Fee, and
Meet all other requirements for admission into the University

Undergraduate Freshman

Qualifications

A Grade 12 student expected to graduate at the end of AY 2019-2020; and those who graduated from K-12 pilot schools and have not enrolled in any technical/diploma/degree programs after graduation with a GWA of not lower than 82%
Passer of PEPT/ALS or NFEA & E Program following DepEd regulations and therefore certified eligible for admission to college/tertiary level

Applicant’s photo (JPEG file)
Grades in English, Math, Science and General Weighted Average (GWA) in Grades 10 and 11
Scanned Grade 10 Report Card (JPEG file)
Scanned Grade 11 Report Card (JPEG file)
Report Cards must clearly show grades in English, Math, Science and GWA

Requirements for Admission

PUPCET 2020 ePermit (downloadable from the PUP Website)
Grade 10 and 11 Certified True Copy of Grades/Original Senior High School Card (F138) with school dry seal
Notarized certification of non-enrolment for senior high school graduates who have not enrolled in any technical/diploma/degree program immediately after graduation
Certificate of Completion for Senior High School
Original PSA Birth Certificate
Certification of Good Moral issued by the SHS Principal/or Guidance Counselor with school dry seal.
Duly authenticated copy of the PEPT/ALS A&E Test Certificate of Rating, certified by the DepEd-NETRC-BALS
School ID (for currently enrolled students) or any valid ID with picture (for previous SHS graduates)

Undergraduate Transferee

Requirements

Requirements for transferring within the PUP System

A recommendation from the Director of the branch where he/she came from
Completed at least two (2) semesters
No failing grade, dropped and withdrawn mark in any academic subject
Met the college academic course/program requirements
Submit the following requirement to the Admission Services:
- Transfer Credential/Honorable Dismissal
- Certification of Grades/Transcript of Records for evaluation purposes
- Receipt of Admission Fee payment
- NSO Copy of Birth Certificate
- Receipt of Admission fee payment
- Curriculum sheet

Requirements for transferring from another school

Pass the psychological test
Completed two (2) semesters or one (1) year (equivalent to 36 units only) for the last two (2) years in any four or five year program/degree/course in the school/university where he/she came from
A weighted average of 2.0 or better with no failed, dropped or withdrawn subjects
Met the college academic program/course requirements
Submit the following requirements:
- Honorable Dismissal/Transfer Credentials
- Transcript of Records
- Certification of good moral character with school/university dry seal
- Course/Subject Description taken from other school/university
- NSO Copy of Birth Certificate
- Medical Clearance from the University Clinic
- Two (2) pcs. 2x2 picture with name tag
- Receipt of Admission fee payment
- Curriculum sheet

Two (2) pieces 2 inch. by 2 inch. (2"x2") picture
One (1) piece 1 inch. by 1 inch. (1"x1") picture (with college background if you stopped for more than 2 years)

Qualifications

ENGLISH, MATH AND SCIENCE must be 85% and above
General Average must be 85% and above
No grade below 82% in other subjects

Accomplished Application Slip (to be downloaded from this page after the assessment of your online registration)
Two (2) pieces 2x2 inch. recent picture with white background and applicants’ name tag at the chest
Original copies of the following documents:
- Form 138 (Grade 10)
- PSA Birth Certificate
- Certificate of Good Moral Character
Photocopy of Certificate of Completion
Place in a clean, long brown envelope with Surname, Given Name and Middle Initial written on the upper left hand corner of the envelope.
LRN (Learner’s Reference Number) must be indicated in the Report Card and Certificate of Completion
ESC (Educational Service Contracting) or QVR (Qualified Voucher Recipient) is a MUST for applicants coming from private schools
Chest X-Ray (film and result)
