Freshmen
Requirements
- Senior High School Report Card (Form 138)
- Form 137A
- Entrance Examination Result
- Certificate of Good Moral Character
- NSO Birth Certificate (photocopy)
- Ishihara Test Result (for Maritime courses only)
- 1×1 pictures with blue background (2 pcs)
- Application Form
Procedures
- Fill out the application form and proceed to the clinic for Ishihara Test (for Maritime courses only)
- Pay the entrance exam fee of P100.00 at the Cashier’s office
- Submit the accomplished form and the official receipt to the Guidance office
- Take the entrance examination. (Note: The entrance examination result will be released the next day)
- Submit the application form with the examination result to the Admission clerk for assessment
- Submit the admission requirements and secure an acceptance letter
- Sign a random testing consent form and anti-fraternity waiver along with the inventory form
Transferees

- Honorable Dismissal/ Transfer Credentials
- Certificate of Good Moral Character
